First Time Doctors

If this is your first time, the following below are required:

• You have until May 1, 2024 to make your reservation.
• The deadline to turn in all other forms to hollyrolland@atsu.edu is May 29, 2024.
• You will also need to fill out the online “Jamaica Dental Mission Form” below.
  • If you have any problems submitting the form, please contact Zachary Rhodes at zacharyrhodes@atsu.edu.

• Form A – The Medical Act Form
• Short-Term Volunteer Form
• Work Permit Exemption Application Form
• Emergency Contact Form
• Certified Copy of Basic Degree Certificate – the one that says “Doctor of Medicine”
• Current License
• Names and Addresses of 2 Medical References
• 2 Photographs (passport size)
• If you were trained at an Offshore Medical School and have a Board Certificate, you will need to submit this.
