Returning Dentists/ Hygienists

If you are returning, the following below are required:

• You have until May 24, 2025 to make your reservation.
• The deadline to turn in all other forms to hollyrolland@atsu.edu is May 24, 2025.
• Please submit all documents listed below using this Submission Form
• You will also need to fill out the online “Jamaica Dental Mission Form” below.
